This 84 yo man on NSAIDs for degenerative joint disease was evaluated for recurrent abd pain and gastrointestinal bleeding. His upper endoscopy, colonoscopy, UGI/SBFT, and abd CT were all negative. a discreet region of small bowel stricturing was identified and resected. On pathology,the segment showed submucosal fibrosis and surface ulceration. Being able to view the entire section in one low power field demonstrates that the segment is narrowed or strictured by mural fibrosis giving a “garden hose� appearance. Both the surgical and pathologic appearance of this small bowel segment are characteristic of Crohn's disease.
